How big should it be?
Rule of thumb: printed width ≥ scanning distance ÷ 10. Pick the surface above and the studio sizes the code for you, then checks the printed module size against what a press can actually hold.

Yes: free forever, at any resolution, with no watermark and no signup. Static codes are generated entirely in your browser, so nothing you encode is ever uploaded.
Never. A static QR code is just your URL encoded as squares: it works as long as your URL works, with no dependence on UseQR.
